Family Medicine Physician
The Family Medicine Physician under the direction of the Chief Medical Officer, will perform full scope family practice in the medical clinic. The Physician will provide quality and cost effective primary care services to the KHP patient population, meeting their diverse health and social needs. The Physician will perform complete examinations of patients and screen all patients for health maintenance needs, in a relationship based model of care with a team of health providers. The Physician will document clinical observations, adhere to industry standards and conduct chart reviews to ensure compliance. The ideal Physician for this position will have the ability to connect respectfully with patients and carry out responsibilities within the mission of KHP. The Physician will ensure all functions of assessments and referrals are done in accordance with all governing regulations and KHP policies, practices, and procedures.
Behavioral Health Counselor
The Mental Health Therapist works as part of an integrated interdisciplinary team serving individuals with behavioral health problems within a primary health care setting. The Mental Health Therapist handles crisis intervention needs, and maintains a daily schedule of individual and/or group therapeutic treatments. In addition to treating severe and persistent mental health issues (SPMI,) they provide counseling and skill training for pain management, diabetic control and other areas of patient education needs. This may include substance abuse and social skills interventions such as areas of domestic violence. In all cases they work closely with primary care providers and other team members to develop specific behavioral change plans for patients that often experience complicated medical needs.
Certified Medical Assistant
The Certified Medical Assistant, under the joint direction of the Clinical Services Manager, Nurse Team Leads and Provider staff, supports an individual Provider or Provider group in the day to day administration of quality patient care.
Nurse Team Lead
The Nurse Team Lead, under direction of the Clinical Services Manager and in conjunction with directives and standing orders set by the Medical Director, provides walk in and phone triage services, nurse visit services, clinical case management, all aimed at enhancing patient-centered care and maximizing outcomes across the patient care continuum. The Nurse Team Lead, in conjunction with the Clinical Services Manager provides oversight to the daily operations of the Medical Assistants assigned to their care team.

The Quality Officer provides strategic and operational leadership to the performance measurement / reporting / improvement initiatives of Klamath Health Partnership (KHP). The Quality Officer provides leadership in the creation of a vision that aligns with KHP and its requirements as a Federally Qualified Health Center (FQHC). The Quality Officer ensures KHP’s performance measurement initiatives align with emerging national and state requirements and opportunities, while maintaining the integrity of the infrastructure and systems that support KHP members in generating and optimizing their use of comparative measures.
