Family Medicine Physician

The Family Medicine Physician under the direction of the Chief Medical Officer, will perform full scope family practice in the medical clinic. The Physician will provide quality and cost effective primary care services to the KHP patient population, meeting their diverse health and social needs. The Physician will perform complete examinations of patients and screen all patients for health maintenance needs, in a relationship based model of care with a team of health providers. The Physician will document clinical observations, adhere to industry standards and conduct chart reviews to ensure compliance. The ideal Physician for this position will have the ability to connect respectfully with patients and carry out responsibilities within the mission of KHP. The Physician will ensure all functions of assessments and referrals are done in accordance with all governing regulations and KHP policies, practices, and procedures.

Dentist

The Doctor of Dental Surgery or Doctor of Medical Dentistry under the direction of the Chief Dental Officer, will perform full scope dentistry in the dental clinic. Nurse Team Lead

The Nurse Team Lead, under direction of the Clinical Services Manager and in conjunction with directives and standing orders set by the Medical Director, provides walk in and phone triage services, nurse visit services, clinical case management, all aimed at enhancing patient-centered care and maximizing outcomes across the patient care continuum. The Nurse Team Lead, in conjunction with the Clinical Services Manager provides oversight to the daily operations of the Medical Assistants assigned to their care team.

Dental Assistant

The Dental Assistant is responsible for a variety of technical duties while assisting the dentist in the examination and treatment of patients. Working chair-side, the Dental Assistant hands instruments and materials to the dentist and dries the patient’s mouth with suction equipment during treatments. The Dental Assistant performs related duties involved in the care and maintenance of dental instruments, equipment and supplies. The Dental Assistant may also mix filling compounds, take impressions of teeth, and provide patients with postoperative instructions.

Medical Records Clerk (Scanning)

The Medical Records Clerk (Scanning) is responsible for collecting all documents from PODs’ out baskets, sorting, scanning all medical records into patient charts, and working out of a queue of scanned documents to appropriately save into patient charts. The Medical Records Clerk is also responsible for answering incoming calls, routing calls to appropriate departments, taking descriptive messages, writing triage requests per triage workflow, and forwarding these to appropriate medical staff. Other duties include: sorting incoming U.S. postal mail, printing refill requests, provider orders or reports that require a provider signature.

Patient Resources Specialist

The Patient Resources Specialist offers assistance to Klamath Health Partnership’s underserved and vulnerable patients – especially those at greater risk for health disparities – in accessing social, financial and other necessary supportive services in order to coordinate the patient’s care with other professionals and improve the patient’s healthcare outcomes.  The Patient Resources Specialist works closely as an ally with the patient to remove barriers to effective care by coordinating services, thus increasing the patient’s chances for a healthier quality of life.  The Marketplace Assister’s main focus is getting insurance coverage for those that are without coverage.

Transportation Driver

The Driver is responsible for transporting patients to and from Klamath Open Door clinics and occasional transports to Medford and Bend. Driver is also responsible for the safety of the passengers while on the road.
